Malignant tumors of the ureteral smooth muscle are rare and the prognosis is not clear. This is a report of a patient with primary leiomyosarcoma of the ureter. An 82-year-old woman was admitted to Department of Surgery, Konyang University Hospital with a palpable mass in right lower abdomen. Abdominopelvic computed tomographic scan demonstrated severe hydronephroureterosis and about 10.5 ¡¿ 10 ¡¿ 7 cm sized huge irregular shaped pelvic mass. Retrograde pyelography showed complete obstruction of the right middle ureter. En bloc resection of the mass and segmental resection of the ureter was performed. 11 months postoperatively, the tumor recurred at right retroperitoneum and metastasized to liver.
